OPERATION

Treatment operation for hypertension

Clinical reports

［摘要］目的：觀察高能生物離子早期治療急性缺血性腦血管病的療效。方法選擇101例急性缺血性腦血管病患者隨機分為兩組：

　?、谥委熃M：在臨床常規治療的基礎上加用高能生物離子治療儀進行治療，時間各為1個月。
觀察指標：

結　論：
　　高能生物離子早期治療急性缺血性腦血管病有肯定療效，治療組無一病例病情惡化，且無副作用，值得臨床進一步觀察和推廣。

Clinical Analysis of Effects on the patients of Cerebral Infarction Treated by the High-energy Bio-ion Instrument Su qingjie Lin teqin Chen jing Wang yuanjiang Chen zhibing(Department of Neurology, Affiliated Hospital of Hainan Medical College, Haikou 570102)
　　Abstract 0bjectives To investigate the effects on the patients of acute cerebral vascular ischemia diseases (ACVID) treated early by the High-energy Bio-ion instrument. Methods 101 cases of ACVID were divided randomly into two groups：

① Controlled group： the patients in this group were treated by the usual therapeutic methods；

② Therapy group：the patients in this group were treated not only by the usual therapeutic methods but also by the treatment with the High-energy Bio-ion　instrument.Observing　Data：① Scores of neurologic deficit；

② The mean velocities (Mvs) of the cerebral arteries. Results Both the numbers of the obvious improvement and the Mvs in the therapy groups were significantly great (P<0.05) compared with the controlled groups. Conclusions：The treatment with the High-energy Bio-ion instrument was one of the effective therapy ways of ACVID.

急性缺血性腦血管?。ˋCVD），是一組以血管源性病變為病因、起病急、引起持續性功能缺損的臨床綜合癥。其發病率高，是中老年人致死、致殘的主要病因之一。為了減少此病的致殘程度，除必要的藥物治療外，目前多采用多種綜合治療措施，包括物理治療。我院利用海南天光高科技開發公司生產的高能生物離子治療儀對急性缺血性腦血管病進行早期治療觀察，觀察結果報告如下。

結　果
2.1治療組55例和對照組46例
患者經不同方法治療1個月后，按療效判斷標準判斷結果見表1：

表1結果顯示治療組基愈、進步、無效及惡化的例數與對照組比較無明顯差異（P>0.05），但顯效例數較對照組增多，統計學有顯著差異（P<0.05）

表2結果提示：治療組在行高能生物離子治療前，大腦中、前、后動脈平均血流流速與對照組比較無顯著性差異（P>0.05），但經過一周的治療后，各腦動脈血流平均流速較治療前及對照組均有明顯提高，具有顯著性差異（P<0.05）。

2.3　副作用：

　　在治療過程中，未發現何副作用

討　論

腦梗死的病理過程中，恢復缺血腦組織的血流供應有利于減輕腦組織進一步損傷壞死及促進關暗帶區瀕死腦組織細胞的修復，而腦動脈血流的平均速度能的反映出腦血流量的大小，而本研究發現高能生物離子早期治療能明顯提高ACVD患者的腦血流速度，因此有利于增加腦血流量無疑有助于對缺血性腦組織損傷的修復，促進神經功能的恢復。

近年來，用物理因子輔助治療腦血管已逐漸引起人們的關注。高能生物離子診斷治療儀治療方法十分獨特，不同于其他心腦血管治療儀，將具有較高抗電能力的治療師與患者構成一個串聯電路進行治療，國內到目前還沒有報道過。我們在臨床治療中觀察到，高能生物離子早期治療ACVD對患者各種功能的改善療效肯定，未見病情惡化和出現副作用的病例。高能生物離子診斷治療儀的確切作用機理尚不十分清楚，我們認為，這種療法可能有如下作用：

　?、僭谔囟ǖ碾妶鲎饔孟?，阻止大量的C 2+ a離子進入神經細胞體內造成超載反應，抑制自由基的產生，防止腦細胞進一步損傷和發生水腫。

　?、陔姶碳ば∧X頂核使局部腦血流量（rCBF）增加，改善損傷區腦組織的供血，促進缺血半暗帶區腦組織功能恢復。從TCD檢測中，我們的確發現經高能生物離子治療后，各腦動脈平均腦血流速度明顯增加，同時可反映出腦血流量的增加。

　?、墼陔妶龅淖饔孟?，改變血液流變學性質，降低血粘度及血小板聚集能力，直接激活紅細胞膜N ﹢a- ﹢K- ATP酶，促進ATP的生成，增加紅細胞變形能力，從而仰制血栓形成，改善微循環。

④中樞神經系統具有高度的可塑性，可塑性是因為腦具有功能重組的能力，腦功能重組的主要機制是突觸的調整和發芽。經顱電刺激可提高神經系統的興奮性，反復的刺激可降低突觸傳導的閾值，使原來不活躍的突觸變為活躍的突觸，從而形成新的傳導通路，
　　另外，局部rCBF的增加，有利于神經纖維的生長，促進軸突側枝長芽，促進各種功能恢復。
結　論
　　高能生物離子診斷治療儀急性缺血性腦血管的療效確切，可明顯減少神經功能缺損評分，增加大腦血流灌注，無副作用，值得在臨床進一步觀察研究的推廣。隨著更深入的研究，高能生物離子診斷治療儀可能具有更廣泛的應用前景。
